
The Division for Ministry has determined that Minister’s Information
Forms, for Non-NA Clergy, cannot be distributed from the Pastoral
Relations Office without the following in his/her file.
1.
Show us
proof of ordination or in-care status.
2.
Copies
of transcripts from all schools.
3.
Completion of a Congregational history and polity course or credit for
its equivalent.
Until a minister completes these requirements, they
are encouraged to request church information forms and
to send their minister’s information form directly to search committees.
A form can be obtained from the Search Committee Chairperson.
